Historically, treatment involves combination of chemotherapy and radiotherapy. Depending on the anatomic location, the role of surgical intervention remains limited. Case Report: A case of an 11-year old girl with Ewing’s sarcoma of the first metacarpal bone of her dominant right hand with a solitary lung metastasis is described. Treatment consisted of neo- and adjuvant chemotherapy, and wide resection of the affected metacarpal bone followed by reconstruction of the thumb with an intercalated autologous second metatarsal bone graft. The bone gap at her left foot was filled with autologous tricortical iliac bone graft. At one year post-surgery, she achieved near normal functions of her dominant hand, with no donor sites morbidity. The surgical procedures are described together with the excellent functional outcomes of the patient. Discussion/ Conclusion: The use of autologous metatarsal bone graft to reconstruct the thumb following tumour resection is an excellent option available.
